APPOINTMENTS

A new patient appointment is a 90-minute session that is scheduled after a brief telephone conversation with Dr. Lee. You are encouraged to bring a trusted family member or friend to your first appointment, as it can be very helpful for Dr. Lee to hear from such a person, though this is not required. Dr. Lee will also provide you with a new patient questionnaire and patient information form which we ask that you complete prior to your first appointment; you may email or fax the completed forms to Dr. Lee, or you may bring them to your first appointment.

Follow-up appointments are 50-minute sessions or 25-minute sessions. Dr. Lee will work with you to determine the appropriate appointment length as well as how frequent the appointments should be. Patients must be seen at least every three months to continue to see Dr. Lee.

FEES

Dr. Lee does not participate in any insurance plans. Patients are responsible for payment on the day of the appointment. Patients with private insurance may file claims for reimbursement with their insurance companies for out-of-network service; Dr. Lee will provide patients with information to allow them to do so but the claims must be submitted by patients directly to the insurance company. Dr. Lee will also be opting out of Medicare; patients with Medicare must therefore agree in writing not to submit claims to Medicare if they wish to be seen by Dr. Lee.

Dr. Lee's fees are similar to those of other psychiatrists in the area. Please contact Dr. Lee for more information.

Fees are due the day of the appointment and can be paid by cash, check, or credit card. Checks can be made out to "Janet Lee, MD." There is a processing charge for returned checks.

MISSED AND LATE APPOINTMENTS

If you are unable to make an appointment, please let Dr. Lee know as soon as possible by calling the office and leaving a voicemail message. Because your appointment time is reserved for you, you will be charged the full appointment fee unless you cancel 48 hours in advance (not including weekends or holidays). Exceptions to the missed appointment fee will be made for weather emergencies. If you are late for your appointment, you will be seen for the remaining appointment time and will be charged the full appointment fee.
